to read and fill out, so please

complete them prior to your visit.  

 

We require that you to complete

your intake questionnaire and forms

in our patient portal which can be found here: 

 

A copy of the Consent to Disclose Confidential Information can be found here.  It is required to correspond with your insurance company and other entities, such as a school, the Department of Human Services, and your medical provider. Please complete it and return it prior to scheduling your first appointment.
